Incident Reporting Procedure & Form

At Ballito United FC, we maintain an environment built on respect, sportsmanship, and safety. This procedure outlines how players, parents, and coaches must handle and report any accidents, injuries, behavioral issues, safety hazards, or breaches of the club’s Code of Conduct.

 

1. Perspective Roles: What is Expected of You?

 

⚽ For Our Players

  • Speak Up: If you are hurt, bullied, feel unsafe, or see a teammate being mistreated on or off the field, tell your coach or your parents immediately.
  • On-Field Disputes: If an incident happens during a match, do not argue with opponents or referees. Let your team captain or coach handle it.

 

πŸ“‹ For Our Parents & Spectators

  • Cool Heads First: If a conflict occurs on the sidelines or on the pitch, do not engage, escalate, or confront opposition parents, players, or match officials.
  • Report Constructively: If you witness a code of conduct breach, a safety hazard at a venue, or if your child reports an issue to you, use the official reporting channels rather than tackling it in the team WhatsApp group.

  • Immediate De-escalation: Your first priority is the physical and emotional safety of the players. Separate conflicting parties, manage the bench, and pause the session if necessary.
  • Mandatory Logging: You are required to submit an official report (below) for any match abandonment, referee dispute, player injury requiring medical care, or serious behavioral breach within 24 hours.

 

2. The 3-Step Reporting Process

 

Step 1: Immediate Action & Care

  • Medical: Administer immediate first aid or call emergency services if required.
  • Safety: Safely remove players from any hostile or hazardous situations.

 

Step 2: Submit the Official Report (Within 24 Hours)

  • Go to the club website and complete the online BUFC Incident Report Form (managed via our Club Administrator).
  • Provide objective, clear facts: Date, time, venue, names of people involved, what happened, and any witness details. Stick to what was seen and heard, avoiding emotional bias.

 

Step 3: Review & Resolution

  • Log & Assess: The Club Administrator logs the incident and sends it to the Club Chairman and disciplinary committee.
  • Investigation: Management will review match cards, gather statements from the coach, parents, or witnesses, and interview the parties involved.
  • Action: The club will issue a resolution within 14 days, which may include formal warning letters, suspension from training/matches of league mediation, , possible permanent suspension from club, or systemic club improvements.

All reports are treated with strict confidentiality in accordance with POPIA.
